Simple Steps for an Easy Admission Process
We would like to make admission to Denton Rehabilitation & Nursing Center a simple, stress-free process. Refer to the guidelines below that fit your situation.
From a Hospital
Please let the Hospital Case Manager know that you have chosen Denton Rehabilitation & Nursing Center as the next step in your recovery. The Case Manager will call us with permission to complete an evaluation. Your clinical information will be reviewed by our admissions committee to determine how best to meet your needs. If accepted, the attending physician at the hospital will approve your discharge from the hospital and write your admission orders to Denton Rehabilitation & Nursing Center.
From a Nursing Facility
Please let the Social Worker at the other facility know that we have permission to see your medical chart. We will review your clinical information and determine how best to meet your needs. If accepted, the attending physician at the other facility will write orders approving your transfer to Denton Rehabilitation & Nursing Center.
From Home
Please request the most recent medical information from your personal physician, including a History & Physical and any recent lab work. We will also need an order from your physician listing your medications, diet, and any other needs. We can come to your home, if needed, to complete an in-person evaluation, and this information will be reviewed by our admission committee to determine how best to meet your needs.
Paperwork
We will need a copy of your insurance cards, and copies of any Advanced Directives such as a Living Will or Power of Attorney. We will also meet with you or your representative to sign our admission paperwork, preferably prior to admission.
